Rebates - Creative including Sound Blaster Audigy2 ZS Platinum

I was not really looking for a new sound card but stumbled across a rebate offer if anyone in the US/Canada is in the market.

The sound cards have a $50USD rebate so this is nothing to sneeze at.

I noticed that some retaillers on the net are offering the rebate but it appears that the following forms are applicable no matter where you purchase the item (forms for all rebated products).
